Уровень гамма-глутамилтрансферазы и NT-proBNP у пациентов с ишемической болезнью сердца и метаболически-ассоциированная жировая болезнь печени

Аннотация

Ишемическая болезнь сердца (ИБС) и метаболически-ассоциированная жировая болезнь печени (МАЖБП) через схожие патогенетические механизмы приводят к высокому риску развития осложнений сердечно-сосудистых заболеваний и смертности. Цель исследования: Изучить уровень гамма-глутамилтрансферазы (ГГТ) и N-концевого пептида натрийуретического гормона (NT-proBNP) у пациентов с ИБС и МАЖБП. Материалы и методы: отобрано 96 пациентов с ИБС в возрасте 37-78 лет, которые были разделены на 2 группы: 1 - лица с ИБС и МАЖБП (n=48); 2 - с ИБС без МАЖБП (n=48). Проанализированы общеклинические показатели; уровни ГГТ и NT-proBNP; индексы FLI, FIB-4; эхокардиография. Результаты: Пациенты с ИБС и МАЖБП чаще имели ожирение, сахарный диабет 2 типа, индекс FLI ≥60 (p=0,002), индекс FIB-4 ≥ 3,25 (p=0,01) в сравнении с пациентами без МАЖБП. Уровень ГГТ был достоверно выше у пациентов 1 группы (p=0,045). В группе ИБС и МАЖБП чаще наблюдалась 2 стадия ХСН (p<0,001), III-IV функциональные классы по NYHA (p=0,002). У лиц 1 группы NT-proBNP был выше, чем у 2 (p=0,003), а фракция выброса левого желудочка была ниже (p<0,001). У пациентов с МАЖБП чаще встречались ХСНнФВ и ХСНунФВ (p=0,001) в сравнении с пациентами без МАЖБП. Выявлены прямые корреляционные связи между уровнем NT-proBNP и ГГТ; NT-proBNP и FLI и FIB-4 у пациентов 1 группы. Заключение: У пациентов с ИБС и МАЖБП установлено достоверное повышение уровней ГГТ и NT-proBNP, что связано с наличием стеатоза печени и разной степени выраженности фиброза печени и ассоциировано с более тяжелым течением ХСН, субтипом ХСНнФВ. Таким образом, у больных с ИБС имеются сложные взаимодействия между сердцем и печенью, особенно при наличии МАЖБП.
