Биологические маркеры риска малигнизации пищевода Барретта

Аннотация

Пищевод Барретта (ПБ) — состояние, при котором происходит замещение многослойного плоского эпителия в дистальном отделе пищевода метаплазированным цилиндрическим эпителием кишечного типа, является факультативным предраковым заболеванием, связанным с повышенным риском развития аденокарциномы пищевода (АКП). Для раннего выявления злокачественной опухоли действующие клинические руководства рекомендуют эндоскопическое наблюдение с обязательным гистологическим исследованием биоптатов слизистой оболочки пищевода. При этом дисплазия эпителия — единственный значимый показатель вероятности возникновения аденокарциномы на фоне ПБ, который сегодня используется в клинической практике. Существующие многочисленные ограничения эффективности такого подхода делают актуальным поиск дополнительных критериев диагностики ранних неопластических нарушений и оценки риска их прогрессии в эпителии ПБ. В рамках данной статьи рассматриваются наиболее значимые генетические и молекулярные нарушения, которые могут претендовать на роль диагностических или предиктивных биологических маркеров при ПБ.
