Саркопения у пациентов с неалкогольной жировой болезнью печени: патогенез, диагностика и лечение (мнение экспертов)

Аннотация

Введение. В настоящее время врачи различных специальностей все чаще стали диагностировать саркопению - заболевание, характеризующееся снижением массы, силы и функции мышц. Саркопения чаще диагностируется у лиц пожилого и старческого возрастов, но может выявляться и у лиц среднего и молодого возрастов как следствие различных заболеваний. Ряд исследований демонстрируют связь саркопении с неалкогольной жировой болезнью печени (НАЖБП). На сегодняшний день остаются нерешенными вопросы диагностики и лечения саркопении в рутинной клинической практике у больных с НАЖБП, а также влияние терапии саркопении на профилактику осложнений и прогрессирование НАЖБП. Методология. Научное общество гастроэнтерологов России (НОГР) объединило 12 экспертов в области изучения саркопении и НАЖБП для вынесения аргументированных положений и рекомендаций по проблеме «Саркопения у пациентов с неалкогольной жировой болезнью печени: патогенез, диагностика и лечение». Расширенный литературный поиск статей на английском языке проводился по ключевым словам - саркопения, классификация саркопении, фенотипы и патогенез саркопении, современные методы диагностики и потенциальные немедикаментозные (рациональное питание и физическая активность) и терапевтические методы коррекции саркопении, неалкогольная жировая болезнь печени в следующих международных базах данных: PubMed, Google Scholar, Web of Science, Embase, Cochrane Database, поиск проводимых актуальных исследований - на сайте ClinicalTrials.gov. Расширенный литературный поиск осуществлялся на русском языке в базах данных: электронный ресурс «Рубрикатор клинических рекомендаций МЗ РФ», eLibrary, RusMed. Согласно полученной информации, текст структурирован в отдельные главы. Каждая глава содержит ключевые положения (мнение экспертов) по соответствующему разделу и собственно имеющиеся на сегодняшний день данные, касающиеся того или иного раздела. Авторами представлены таблицы, выводы экспертов и список современной литературы, посвященной саркопении и ее фенотипам. Результаты (по ключевым положениям). Эксперты работали над определением наиболее значимых проблем по указанной теме, сформулировали актуальные для клинической практики вопросы и дали на них аргументированные ответы, оформленные как «положения» для клинической практики с комментариями на основе доказательной медицины. В основу формулировки вопросов, рекомендаций и положений лег критический анализ медицинской литературы в англоязычных и русскоязычных базах данных. Результаты работы экспертов имеют прямое отношение к качественному ведению пациентов с саркопенией и НАЖБП, сформулированные ими ключевые положения могут использоваться в клинической практике. Выводы. Представляемый группой экспертов документ «Саркопения у пациентов с неалкогольной жировой болезнью печени: патогенез, диагностика и лечение» призван помочь практическим врачам улучшить знания по проблеме саркопении и научиться рано диагностировать синдром в рутинной клинической практике, причем не только у пациентов старших возрастных групп, но и у больных молодого и среднего возраста, у кого саркопения развивается как следствие хронических заболеваний.
