Ранний артрит и заболевания органов пищеварения. Взгляд ревматолога

Периферический артрит относится к числу самых частых проявлений суставного синдрома, не всегда связанного с ревматическими заболеваниями. Определение его нозологической принадлежности и рациональная терапия на ранней стадии (ранний артрит) представляют сложную клиническую и диагностическую проблему, требующей междисциплинарного подхода. В обзоре представлены вопросы дифференциальной диагностики раннего артрита при таких заболеваниях желудочно-кишечного тракта как воспалительные заболевания кишечника, целиакия, синдром РРР (Панкреатит Паникулит и Полиартрит), Pancreatitis, panniculitis and polyarthritis (PPP-) syndrome) и др., проявляющиеся сходной клинической симптоматикой с такими ревматическими заболеваниями как ранний ревматоидный артрит и спондилоартриты. Большинство представленных в обзоре иммунновоспалительных заболеваний мультисистемны и гетерогенны по клинической симптоматике, течению и исходам. Поэтому диагностический процесс, особенно при раннем артрите с типичным многосистемным вовлечением, носит комплексный познавательный характер и требует синтеза многих данных, обычно не учитываемых в простом алгоритме, основой которого является набор классификационных критериев.
