Системное воспаление и кардиоваскулярные риски у больных воспалительными заболеваниями кишечника: что необходимо учитывать?

Аннотация

Несмотря на продолжающийся рост заболеваемости воспалительными заболеваниями кишечника (ВЗК), наблюдается увеличение продолжительности жизни у больных язвенным колитом (ЯК) и болезнью Крона (БК), что связывают с применением современных методов лечения. Лечение ВЗК стало более эффективным благодаря появлению биологических методов терапии, расширению доступа к специализированной помощи и подходу «treat to target». Однако, с увеличением продолжительности жизни больных ВЗК происходит и рост числа лиц с коморбидностью, в первую очередь с сочетанием ВЗК с кардиоваскулярной патологией. Факторы окружающей среды приводят к изменению разнообразия и плотности заселения микробиоты кишечника, нарушению его барьерной функции, иммунной дисрегуляции и развитию хронических воспалительных заболеваний и атеросклероза. При ВЗК повышается уровень провоспалительных цитокинов, С-реактивного белка и гомоцистеина, что приводит к дисфункции эндотелия и атеросклерозу. Кроме того, воспалительные процессы при ВЗК способствуют гиперкоагуляции, которая имеет место как в возникновении тромбоэмболических осложнений , так и в патогенезе самого заболевания. Предполагают, что медикаментозная патогенетическая терапия ВЗК также связана с риском сердечно-сосудистых заболеваний. В представленном обзоре мы систематизируем имеющиеся данные в отношении рисков кардиоваскулярных заболеваний у больных ВЗК. Поиск литературы, содержащей информацию о соответствующих исследованиях, проводился в системах PubMed и Google Scholar по ключевым словам: воспалительные заболевания кишечника, сердечно- сосудистые заболевания, воспаление, атеросклероз.
