Оценить функциональное состояние печени и билиарного тракта у детей с сахарным диабетом I типа и их сопряженность с продолжительностью заболевания и выраженностью нарушений метаболических процессов. Материалы и методы. Обследовано 74 пациента с сахарным диабетом I типа в возрасте от 7 до 17 лет. Сформировано 2 группы: 1 группа - дети с продолжительностью заболевания до 5 лет (33 человека), 2 группа - с продолжительностью заболевания более 5 лет (41 человек). Изучено функциональное состояние гепатобилиарной системы на основании клинических, лабораторных, инструментальных показателей. В случае выявления изменений со стороны печени по данным ультразвукового исследования - рассчитывался индекс стеатоза печени. Результаты. С увеличением давности заболевания повышалось количество больных с изменением эхогенности и однородности структуры паренхимы печени: повышение эхогенности печени выявлялось в 1 группе в 24,2% случаев, во 2 группе - в 36,6% (р&lt;0,05). Показатели индекса стеатоза печени, характерные для неалкогольной жировой болезни печени, чаще выявлялись при давности заболевания более 5 лет - 17,1% против 6,1% (р&lt;0,05). Биохимические показатели пациентов сохранялись в пределах нормы за исключением показателей щелочной фосфатазы (ЩФ) и гамма-глутамилтранспептидазы (ГГТП), достоверно повышающихся при давности сахарного диабета более 5 лет (433,91±205,01 и 411,05±220,32 ЕД/л; 16,22±7,07 и 9,74±3,56 ЕД/л).</p></abstract><trans-abstract xml:lang="en"><p>The aim was to assess the functional state of the liver and biliary tract in children with diabetes mellitus type I and their correlation with the duration of the disease and the severity of metabolic changes. Materials and methods. There were examined 74 patients with diabetes mellitus type I aged from 7 to 17 years. There were formed 2 groups: 1 group - children with a disease duration up to 5 years (33 people), 2 group - with a disease duration of more than 5 years (41 people). There were studied the functional state of the hepato-biliary system on the basis of clinical, laboratory and instrumental parameters. Results. With an increase in the duration of the disease, increased the number of patients with a change in the echogenicity and homogeneity of the liver parenchyma: an increase in liver echogenicity was detected in group 1 in 24.2% of cases, in group 2 - in 36.6% (p&lt;0.05). The fatty liver index characteristic of non-alcoholic fatty liver disease were more often detected when the disease lasted more than 5 years - 17.1% versus 6.1% (p&lt;0.05).
