Цель исследования: выявить особенности клинической, эндоскопической и морфологической картины у детей с ГЭРБ с подозрением на пищевод Барретта по данным ЭГДС. Материалы и методы: Первичную группу исследования составили 87 детей, страдающих ГЭРБ. По результатам ЭГДС и гистологического исследования биоптатов слизистой оболочки пищевода у 13 из 87 пациентов были выявлены признаки метаплазии. Эти пациенты составили основную группу исследования. Результаты: У 13 из 87 пациентов с ГЭРБ (14,9%) была выявлена метаплазия слизистой оболочки пищевода (пищевод Барретта): в 10 случаях (11,5%) - по желудочному типу, в 3 (3,4%) - по кишечному типу. По данным ЭГДС, у 6 детей пищевод Барретта был выявлен на фоне эрозивной формы ГЭРБ и у 7 - на фоне неэрозивной ГЭРБ, причем кишечная метаплазия отмечалась только у детей с эрозиями в пищеводе. Клиническая картина у больных с метаплазией эпителия слизистой оболочки пищевода не отличалась от таковой у детей без метаплазии. По данным суточного мониторинга рН у пациентов с пищеводом Барретта в 53,8% случаев определялся физиологический ГЭР, в 38,5% - патологический кислый ГЭР и у одного ребенка (7,7%) - патологический некислотный ГЭР. При этом у всех детей с кишечной метаплазией определялся патологический кислый ГЭР. Выводы: У детей с ГЭРБ возможна метаплазия слизистой оболочки пищевода как по желудочному, так и по кишечному типу. Длительное течение эзофагита, эрозивный эзофагит, патологический кислотный ГЭР предрасполагают к развитию ПБ. Достоверных клинических особенностей течения ПБ у детей с ГЭРБ выявлено не было.

Study Aim. To study clinical, endoscopic and morphological patterns in children with GERD suspected Barrett’s esophagus according endoscopy. Methods. The study involved 87 children with GERD. According to the results of endoscopy and histological examination of biopsy specimens of esophageal mucosa in 13 of 87 patients were identified signs of metaplasia. Results. In 13 of 87 patients with GERD (14.9%) were detected metaplasia of the esophagus (Barrett’s esophagus): in 10 cases (11,5%) - gastric, in 3 (3,4%) - intestinal type. According to EGD, 6 children with Barrett's esophagus had erosive GERD, and 7 - non-erosive GERD. Intestinal metaplasia was detected only in children with erosions in the esophagus. Clinical manifestations of GERD in patients with metaplasia did not differ from that in children without metaplasia. According to the 24-hour intragastric acidity, 53.8% of children with Barrett's esophagus had physiological GER, 38.5% of pathological acidic GER and one child (7,7%) - abnormal non-acid GER. All children with intestinal metaplasia had pathologic acidic GER. Conclusions. Not revealed any clinical features of Barrett's esophagus in children with GERD. Long term GERD, erosive esophagitis, pathological acid GER - predisposing factors of Barrett's esophagus.
