Цель. Проанализировать акушерско-гинекологический, инфекционный и соматический статус матерей у недоношенных детей, для оптимизации тактики наблюдения в постнатальном периоде. Материалы и методы. В исследование включено 60 детей, рожденных преждевременно с определением особенностей акушерско-гинекологического, инфекционного и соматического анамнеза их матерей методом ретроспективного анализа. В зависимости от гестационного возраста (ГВ) дети были подразделены на 3 группы: 1 группа - ГВ 34,1-36,9 недель (20 детей); 2 группа - ГВ 30,1-34 недели (21 ребенок); 3 группа - ГВ 26-30 недель (19 детей). Результаты. Наиболее выраженная отягощенность акушерско-гинекологического и соматического анамнеза имела место у матерей во 2й и 3й группах, где достоверно чаще встречались рождение недоношенных детей (23,8%; 31,6%) и прерывание беременности (28,6%; 57,9%) в анамнезе. Антенатальная гибель плода имела место только у матерей во 2й и 3й группах (4,8%; 10,5%). Максимальная отягощенность по воспалительным заболеваниям половой сферы имела место 3й группе. Инфекционная патология других систем органов, представленная острым гестационным или хроническим пиелонефритом в стадии обострения, тонзиллитом, бронхитом значительно чаще встречалась у пациентов в 3й группе (52,6%, 47,4%, 21,1%). Оперативное родоразрешение в исследуемых группах проводилось достаточно часто: 45% в 1 группе, 47,6% - во 2 группе, 31,6% - в 3 группе, в целом - 41,7% от общего количества матерей (в том числе, у 16% показанием к кесареву сечению была преждевременная отслойка плаценты). Заключение. Таким образом, изучение перинатального анамнеза выявило наличие широкого спектра патологических факторов, агрессивно действующих на плод, оказывая как непосредственное повреждающее действие на ткани, так и нарушая функциональную активность различных систем органов.</p></abstract><trans-abstract xml:lang="en"><p>Of particular clinical importance at present is a detailed study of perinatal history, since the pathological obstetric-gynecological, infectious and somatic status of the mother causes dysfunctions in the fetoplacental complex, increases the risk of premature birth, disrupts the vital functions of the fetus and reduces the reserve capabilities of all organ systems in the newborn. Purpose. Analyze obstetric-gynecological, infectious and somatic status of mothers in premature babies, to optimize postnatal observation tactics. Materials and methods. The study included 60 children born prematurely with the determination of the features of obstetric-gynecological, infectious and somatic history of their mothers by retrospective analysis. Depending on gestational age (GV), children were divided into 3 groups: 1 group - GV 34.1-36.9 weeks (20 children); Group 2 - GV 30.1-34 weeks (21 children); Group 3 - GV 26-30 weeks (19 children). Results. The most pronounced burden of obstetric-gynecological and somatic history occurred in mothers in the 2nd and 3rd groups, where the birth of premature babies was significantly more common (23.8%; 31.6%) and abortion (28.6%; 57.9%) in the history. Antenatal fetal death occurred only in mothers in the 2nd and 3rd groups (4.8%; 10,5%). The maximum burden for inflammatory diseases of the sexual sphere occurred in the 3rd group. Infectious pathology of other organ systems, represented by acute gestational or chronic pyelonephritis in the stage of exacerbation, tonsillitis, bronchitis, was significantly more common in patients in the 3rd group (52.6%, 47.4%, 21.1%). Operative delivery in the study groups was quite frequent: 45% in group 1, 47.6% in group 2, 31.6% in group 3, in general - 41.7% of the total number of mothers (including 16% indication for caesarean section had premature placental detachment). Conclusion.
