Развитие лапароскопической хирургии основанное на технологическом прогрессе и накоплении клинического опыта идет строго в направлении минимизации инвазивности вмешательств, уменьшении травматизма операционного доступа и снижения числа послеоперационных осложнений. Цель работы - представить результаты применения методики единого лапароскопического доступа (ЕЛД) у пациентов с ожирением. Материал и методы. Лапароскопические операции по методике ЕЛД, выполнены у 27 пациентов с избыточной масса тела и ожирением. Все операции выполнялись в плановом порядке по поводу хронического калькулезного холецистита. Результаты и обсуждение. Нами применены вспомогательные технологии для тракции органов (Патент РФ N 103722) при возникновении «конфликта» инструментов, что позволило сократить время вмешательства. Среднее время вмешательства составило 55,5±6,9 мин. У 10 пациентов с факторами риска образования троакарных грыж (ожирение, пожилой возраст, функциональная недостаточность соединительной ткани) область доступа превентивно укреплялась уложенным поверх апоневроза имплантом. Все пациенты выписаны в удовлетворительном состоянии на 3-4 сутки после операции. В последующем, при наблюдении и повторных осмотрах в сроках до 7 лет после операции, пациенты жалоб, связанных с хирургическим вмешательством не предъявляли, чувствовали себя хорошо, и не зафиксировано ни одного случая образования послеоперационной вентральной грыжи. Заключение. Очевидны преимущества холецистэктомий у пациентов с избыточной массой тела и ожирением по методике ЕЛД, связанные со снижением травматичности операции, превосходным косметическим результатом и достижением принципиально иного качества жизни пациентов.

The development of laparoscopic surgery, based on technological progress, and the accumulation of clinical experience makes it possible to reduce operating access, reducе the postoperative complications. Material and methods. SILS cholecystectomy were performed in 27 patients with overweight and obesity. All operations were done as planned for chronic calculousis cholecystitis. Results and discussion. We used assistive technologies for organ traction (Patent N 103722 Russian Federation) in the event of a “conflict” of instruments, which made it possible to reduce the intervention time. The mean intervention time was 55.5±6.9 minutes. In 10 patients with risk factors for the formation of trocar hernias (obesity, advanced age, functional insufficiency of the connective tissue), the access area was preventively strengthened by an implant placed over the aponeurosis. All patients were discharged in a satisfactory condition 3-4 days after surgery. Subsequently, during observation and re-examinations up to 7 years after surgery, patients did not complain about surgical intervention, felt well, and not a single case of postoperative ventral hernia was recorded. Conclusion. The advantages of cholecystectomy in overweight and obese patients using the SILS technique are obvious, associated with a reduction in the invasiveness of the operation, an excellent cosmetic result, and the achievement of a fundamentally different quality of life for patients.
