Определить уровень экспрессии мотилина и вазоактивного кишечного пептида (ВИП) в слизистой оболочке сигмовидной кишки у пациентов с дивертикулярной болезнью толстой кишки (ДБТК) и синдромом раздраженного кишечника (СРК). Материал и методы. Обследовано 120 человек: 85 больных с ДБТК и 35 - с СРК. Больные были распределены на 3 группы: 1-я группа - 40 пациентов с неосложненной ДБТК с клиническими проявлениями (НДБТК - КП); 2-я группа - 45 человек с бессимптомным течением ДБТК; 3-я группа больных была представлена лицами с СРК с запором (СРКз). Группу контроля составили 30 практически здоровых человек. Все пациенты и здоровые обследованы по единой программе, включающей клинические данные, лабораторные (общий анализ крови с лейкоцитарной формулой, С-реактивный белок), инструментальные (колоноскопия, ультразвуковое исследование органов брюшной полости) данные. Для верификации экспрессии гормонов использовались первичные антитела к мотилину и ВИП. Результаты. Клиническая картина у лиц с НДБТК-КП соответствовала классическим проявлениям СРК с абсолютным доминированием запора у 87,5% больных. Иммуногистохимический анализ показал значительное снижение уровня экспрессии мотилина и ВИП у пациентов в данной группе больных. Группа с НДБТК-КП была статистически неразличима с группой СРКз (p &gt;0,05), однако статистически значимо отличались от группы с бессимптомным течением ДБТК (p &lt;0,05). Выводы. Полученные данные свидетельствуют о важной роли экспрессии мотилина и ВИП в манифестации клинических проявлений ДБТК и СРК и дают основание предположить участие данных гормонов не только в моторно-эвакуаторных нарушениях, но и пролиферативных процессах в слизистой оболочке толстой кишки, которые, возможно, инициируют деструктивные процессы, сопровождающиеся формированием дивертикулов.</p></abstract><trans-abstract xml:lang="en"><p>Purpose of the study. To determine the level of expression of motilin and vasoactive intestinal peptide (VIP) in the mucosa of the sigmoid colon in patients with diverticular disease (DD) of the colon and irritable bowel syndrome (IBS). Material and methods. 120 people were examined: 85 patients with DD and 35 with IBS. The patients were divided into 3 groups: group 1-40 patients with symptomatically uncomplicated DD (SUDD); group 2-45 people with an asymptomatic form of DD; the 3rd group of patients was represented by persons with IBS with constipation (IBSc). The control group consisted of 30 practically healthy people. All patients and healthy people were examined according to a single program, including clinical data, laboratory (general blood count with leukocyte formula, C-reactive protein), instrumental (colonoscopy, ultrasound examination of the abdominal organs) data. Primary antibodies to motilin and VIP were used to verify hormone expression. Results. The clinical picture in individuals with SUDD corresponded to the classical manifestations of IBS with the incidence of constipation in 87.5% and diarrhea in 12.5% of patients. Immunohistochemical analysis showed a significant decrease in the expression level of motilin and VIP in patients in this group of patients. The group with SUDD was statistically indistinguishable from the group with IBSc (p &gt; 0.05), however, it was statistically significantly different from the group with asymptomatic DD (p &lt; 0.05). Conclusions.
