Однако метод ESD технически сложный процесс, сопровождающийся высоким риском операционных осложнений.Цель исследования: модифицировать методику тракции при ESD и оценить ее эффективность.Материалы и методы: экспериментальное сравнительное исследование на биологических препаратах (свиные желудки). Выполнено 144 ESD тремя различными методами: 1 группа — метод стандартной ESD (n-48); 2 группа — ESD с методикой тракции с использованием клипсы и нити (n-48); 3 группа — ESD с модифицированной методикой тракции опухоли (n-48).Результаты: наименьшее время диссекции подслизистого слоя было в 3-й группе 4,89 [2–12] мин, в 1-й и 2-й группах 13,27[4–31] мин и 9,37 [4–26] мин, соответственно, (p = 0,05). Общее время операции в 3-й группе составило 21,10 [12–31] мин, в 1-й—23,85 [12–48] мин и 2-й—25,56 [13–49] мин, p = 0,002. Максимальная скорость диссекции составила в 3-й группе 0,48 [0.20–1.27] (мин/см2), во 2-й и 3-й — 0.94[0.47–2.06] (мин/см2) и 1.48 [0.56–4.10] (мин/см2), соответственно, p=0,003. Показатель HM0 в 1-й группе—89,6% [43/48], во 2-й—93,7% [45/48] и в 3-й—100% [48/48], p&gt;0,05. Расслоение мышечных волокон в ходе диссекции подслизистого слоя в 1-й группе—25% [12/48], во 2-й—в 6.25% [3/48] и в 3-й—12.5% [6/48], p=0,389.Заключение: модифицированная методика тракции опухоли при эндоскопической подслизистой диссекции позволяет лучше визуализировать подслизистый слой, увеличить скорость диссекции, тем самым сокращая общее время операции и время диссекции подслизистого слоя.</p></abstract><trans-abstract xml:lang="en"><p>Background: the method of endoscopic submucosal dissection (ESD) allows to remove large neoplasms of the GI tract enblock, which significantly reduces the risk of local tumor recurrence. However, ESD method is technically complicated with high risk of surgical complications.Aim: to modify ESD traction technique and evaluate its efficiency.Materials and methods: experimental comparative study on biological preparations (pig stomachs). 144 ESD with three different methods were performed: the 1st group—standard ESD method (n-48); the 2nd—ESD with the method of traction with the clip and thread (n-48); the 3rd—ESD with a modified method of tumor traction (n-48).Results: the shortest time to submucosal dissection was 4.89 min [2–12] in the 3rd group, and 13,27 [4–31] min and 9,37 [4–26] min in the 1st and 2nd, respectively (p = 0,05). Total operative time in the 3rd was 21,10 [12–31] min, in the 1st 23.85 [12–48] min, and in the 2nd—25.56 [13–49] min, p = 0.002. The max dissection speed was 0.48 [0.20–1.27] (min/cm2) in the 3rd group, 0.94 [0.47–2.06] (min/cm2) and 1.48 [0.56–4.10] (min/cm2) in the 2nd and 3rd, respectively, p=0.003. HM0 was 89.6% [43/48] in group 1, 93.7% [45/48] in the 2nd, and 100% [48/48] in the 3rd, p&gt;0.05.
