Микробиологический мониторинг условно- патогенной микрофлоры пищеварительного тракта у недоношенных новорожденных с различной степенью дефицита массы тела для обоснования групп риска и возможности персонифицированного подхода к ведению таких пациентов.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В обсервационное исследование случай- контроль включены 67 недоношенных новорожденных (35 мальчиков и 32 девочки), разделенных на три группы в соответствии со степенью дефицита массы тела: низкой (n=21), очень низкой (n=21) и экстремально низкой (n=25) массой. Проведено комплексное обследование клинико-анамнестическими, лабораторными и клинико- функциональными методами диагностики, консультации невролога, офтальмолога, кардиолога. Микробиологический мониторинг условно- патогенной микрофлоры (УПМ) полости рта и других биотопов (поверхность ушной раковины, подмышечная впадина, плодная часть плаценты) проводился в 1-е, 3-и и 7-е сутки жизни. Биохимическая идентификация производилась с использованием тест-систем “Erba Lachema”. Создание базы данных и статистическая обработка проводились с использованием пакетов программ Microsoft® Offi ce® 2010, IBM® SPSS® Statistics 23.0, WinPEPI© 11.39.</p></sec><sec><title>Результаты</title><p>Результаты. Частота выделения УПМ из полости рта новорожденных с дефицитом массы тела была достоверно выше в 1-е и 3-и сутки (точный тест Фишера, р = 0,037) и характеризовалась более широким спектром по сравнению с другими исследованными биотопами. Доминирующим представителем УПМ в большинстве биотопов являлся S. epidermidis, а также K. pneumoniae, C. krusei, Burkholderia cepacia complex и S. maltophilia. Выявлена корреляционная связь между качественными и количественными параметрами УПМ, а также числом контаминированных УПМ биотопов с уровнем массы тела и показателями резистентности недоношенных новорожденных с дефицитом массы.</p></sec><sec><title>Заключение</title><p>Заключение. Микробиологический мониторинг УПМ биотопа полости рта и внутренней поверхности ушной раковины позволяет рассматривать эти микроэкологические параметры в качестве прогностического критерия течения и исходов у недоношенных новорожденных с дефицитом массы. Дает возможность научно обосновать персонифицированный подход к ведению таких пациентов, формированию групп риска, назначению коррекционных и превентивных мероприятий для создания адекватного профиля микробного заселения, профилактики инфекционной патологии и снижения риска неблагоприятного исхода.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>The aim</title><p>The aim. Microbiological monitoring of the digestive tract opportunistic microfl ora in underweight premature newborns to substantiate risk groups and the possibility of a personalized approach to manage such patients.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. A case-control observational study included 67 preterm infants (35 boys and 32 girls), divided into three groups according to the underweight degree: low (n = 21), very low (n = 21) and extremely low (n = 25) mass. A comprehensive examination was carried out with clinical- anamnestic, laboratory and clinical- functional diagnostic methods, consultation of a neurologist, ophthalmologist, cardiologist. Microbiological monitoring of opportunistic microfl ora (UPM) of the oral cavity and other biotopes (auricle surface, armpit, fetal part of the placenta) was carried out on the 1st, 3rd and 7th days of life. Biochemical identifi cation was carried out using test systems “Erba Lachema”. Database creation and statistical processing were carried out using software packages of Microsoft® Offi ce® 2010, IBM® SPSS® Statistics 23.0, WinPEPI© 11.39.</p></sec><sec><title>Results</title><p>Results. The frequency of opportunistic microfl ora isolated from the oral cavity of underweight newborns was signifi cantly higher on the 1st, 3rd days (Fisher’s exact test, p = 0.037) and was characterized by a wider spectrum compared to other studiedbiotopes. The dominant representative of opportunistic microfl ora in most biotopes was S. epidermidis, as well as K. pneumoniae, C. krusei, Burkholderia cepacia complex, and S. maltophilia. There was revealed a correlation link between the qualitative and quantitative parameters of opportunistic microfl ora, as well as the number of opportunistic microfl ora contaminated biotopes with the level of body weight and resistance indicators of underweight premature newborns.</p></sec><sec><title>Conclusion</title><p>Conclusion. Microbiological monitoring of opportunistic microfl ora of the oral cavity and the inner surface of the auricle biotope makes it possible to consider these microecological parameters as a prognostic criterion for the course and outcomes in premature infants with underweight, as well as to scientifi cally substantiate a personalized approach to the management of such patients, the formation of risk groups, the appointment of corrective and preventive measures to form adequate profileof microbial colonization, prevent of infectious pathology and the decrease risk of an unfavorable outcome.</p></sec></trans-abstract><kwd-group xml:lang="ru"><kwd>микробиота</kwd><kwd>недоношенные новорожденные</kwd><kwd>дефицит массы тела</kwd><kwd>условно-патогенная микрофлора</kwd><kwd>S. epidermidis</kwd></kwd-group><kwd-group xml:lang="en"><kwd>microbiota</kwd><kwd>premature newborns</kwd><kwd>underweight</kwd><kwd>opportunistic microfl ora</kwd><kwd>S. epidermidis</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Sassone- Corsi M., Raff atellu M.
