При эндоскопическом исследовании в верхней трети пищевода была выявлена короткая рубцовая стриктура 3 степени, а в нижележащих отделах слизистая пищевода на протяжении 16 см была представлена цилиндрическим эпителием. Гистологическое исследование верифицировало длинный сегмент пищевода Барретта. Выше стриктуры среди неизмененного плоского эпителия пищевода определялись крупные участки гетеротопии желудочной слизистой. На фоне приема ингибиторов протонной помпы пациенту был проведен длительный курс эндоскопического бужирования с интрамуральными инъекциями дексаметазона, что позволило стабилизировать просвет в зоне стриктуры на уровне 12 мм. Тщательный осмотр всех отделов пищевода с применением современных уточняющих методов визуализации позволяет правильно интерпретировать эндоскопическую картину и определить дальнейшую тактику лечения и динамического наблюдения с учетом возможных рисков развития неоплазии в гетеротопированном участке слизистой верхней трети пищевода.</p></abstract><trans-abstract xml:lang="en"><p>A 26-year-old patient with no symptoms of gastroesophageal reflux disease developed complaints of dysphagia. Endoscopic examination revealed a short grade 3 cicatricial stricture in the upper third of the oesophagus, and in the lower parts of the oesophagus mucosa was represented by a columnar epithelium for 16 cm. Histological examination verified the long segmental Barrett’ oesophagus. Above the stricture large areas of heterotopia of the gastric mucosa were determined among the regular oesophageal squamous epithelium. While taking proton pump inhibitors, the patient underwent a long course of endoscopic bougienage with intramural dexamethasone injections, which made it possible to stabilize the lumen in the stricture zone at a level of 12 mm.
