Учитывая наличие гигантской постнекротической полости с большим количеством секвестров (диагностированной по данным инструментальных методов исследования—КТ, УЗИ), было принято решение о применении сочетания малотравматичных способов секвестрэктомии: эндоскопической транслюминальной секвестрэктомии (ЭТС) с чрескожным дренированием под УЗ-наведением и видеоассистированной ретроперитонеоскопической секвестрэктомией (ВАРС).</p><sec><title>Обсуждение</title><p>Обсуждение: Острый деструктивный панкреатит остается актуальной хирургической проблемой в связи с сохраняющимся значительным количеством разнообразных осложнений и летальных исходов, несмотря на достигнутые успехи в лечении данной группы пациентов. В настоящее время опубликовано большое количество работ, посвященных использованию современных малотравматичных способов в лечении инфицированного панкреонекроза. Кроме того, в ряде случаев, в лечении указанной группы пациентов описано применение комбинированных методик с целью наиболее эффективного выполнения секвестрэктомии (в том числе комбинации малотравматичных способов секвестрэктомии).</p></sec><sec><title>Вывод</title><p>Вывод: Использование комбинации малотравматичных способов секвестрэктомии в лечении деструктивного панкреатита является эффективной хирургической тактикой, позволяющей улучшить результаты проводимого лечения.</p></sec></abstract><trans-abstract xml:lang="en"><p>This article presents the clinical case of using combination of minimally invasive approaches of debridement in patient with infected necrotizing pancreatitis. The decision of using minimally invasive approaches: endoscopic transluminal pancreatic necrosectomy combined with percutaneous catheter drainage under US-navigation and video-assisted retroperitoneal debridement was made, due to the giant necrotic cavity, containing huge amount of necrotized tissues (diagnosed by instrumental methods—CT, US).</p><sec><title>Discussion</title><p>Discussion: Acute necrotizing pancreatitis is still the important surgical problem, because of the great number of different complications and high level of mortality, despite achievements in the treatment of this group of patients. Many articles, focused on the use of modern minimally invasive approaches in case of infected necrotizing pancreatitis, have been published.
