Определение основных закономерностей нарушения обмена жирных кислот у новорожденных с сепсисом.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Содержание жирных кислот в сыворотке крови анализировали с помощью метода тандемной масс-спектрометрии на аналитическом приборе Quattromicro MSMS (PerkinElmer, Финляндия).</p></sec><sec><title>Критерии включения</title><p>Критерии включения. Группа 1 — новорожденность, наличие инфекционно-воспалительного очага и дисфункция двух систем и более (сепсис), сопоставимая тактика антибактериальной терапии для новорожденных с сепсисом. Группа 2 — новорожденные с пневмонией, наличие инфекционно-воспалительного очага в легких и отсутствие дисфункции органов и систем, требующих проведения заместительной терапии. Группа 3 — здоровые новорожденные проходившие обследование в Перинатальном центре г. Уфы.</p></sec><sec><title>Результаты</title><p>Результаты. В группе 1 (сепсис) в сравнении с группой 2 (пневмония) статистически значимо были увеличены ЖК С4 и статистически значимо снижены С6, С8, С10, С14, С16, С18. При сравнении группы 1 (сепсис) с группой 3 (здоровые) статистически значимо были снижены ЖК с длинной цепью С16, С18 и повышены ЖК С14, С10, С8, С6, С4. Сравнение групп 2 и 3 показало увеличение содержания ЖК С2, С3, С6, С8, С14 и снижение С18. Содержание ненасыщенных жирных кислот в группе 1 при сравнении с группой 2 статистически значимо были повышены и С5:1, С8:1, С 14:1, С14:2, С16:1, С18:1. Аналогичная картина наблюдалась при сравнении 1-й группы и 3-й. Статистически значимо в 1-й группе были повышены С5:1, С8:1, С10:1, С14:1, С14:2, С16:1, С18:1, С18:2. У детей с сепсисом отмечается повышение содержания ненасыщенных жирных кислот в крови по сравнению с детьми с пневмонией (группа 2) и здоровыми новорожденными. В группе детей с пневмонией по сравнению со здоровыми новорожденные статистически значимо были повышены сывороточные уровни С5:1, С10:1, С14:2, С14:ОН, С18:2 и снижены концентрации С8:1, С14:1, С16:1.</p></sec><sec><title>Заключение</title><p>Заключение. Нарушение метаболизма жирных кислот у новорожденных с сепсисом характеризуется повышением ненасыщенных жирных кислот, что, по-видимому, и характеризует метаболический профиль полиорганной дисфункции.</p></sec></abstract><trans-abstract xml:lang="en"><p>The aim of our study was to develop a new scale for predicting clinical outcome in newborns with sepsis, taking into account the information about depth of metabolic disorders.</p><sec><title>Methods</title><p>Methods. Design of study — retrospective observational single-center study. Medical cards data of 163 newborns with sepsis were included in analysis. To measure the predictive value of the analyzed clinical and laboratory signs we used the Kullback measure. The clinical outcome of the disease was determined as the response function: survived or died.</p></sec><sec><title>Results</title><p>Results. An analysis of the predictive value of the clinical and laboratory parameters of in newborns with sepsis was made, the threshold values of the most informative indicators were developed, which were: the number of blood platelets, the level of total blood protein, body weight and the number of blood neutrophils. The Clinical and Laboratory Condition Index for Newborns scale has been developed, which takes into account the depth of metabolic disorders. ROC analysis (Area Under Curve — 0,723) and the information method (information coefficient 0.992) showed that the Clinical and Laboratory Condition Index score can be used in the intensive care unit to predict the risk negative clinical outcome and evaluate the effectiveness of treatment in infants with organ dysfunction caused by systemic infection.</p></sec><sec><title>Conclusion</title><p>Conclusion.
