Аллергические реакции на пищевые антигены могут протекать по любому из 4 основных типов иммунопатологических реакций. Наиболее характерно сочетание нескольких типов реакций у одного больного. Изучен спектр пищевой сенсибилизации у пациентов с болезнями органов пищеварения (БОП) для корректировки их диетической терапии. Проведено клинико-лабораторное обследование и лечение 205 пациентов: 180 пациентов с БОП (гастроэзофагеальная рефлюксная болезнь, хронический гастродуоденит, дискинезия желчевыводящих путей, синдромом раздраженного кишечника, воспалительные заболевания кишечника (ВЗК), язвенный колит (ЯК), болезнь Крона) и 25 здоровых взрослых. Для выявления реакций I, III и IY типа с пищевыми аллергенами использовали комплекс методов - ИФА (IgE специфические) и РТМЛ в модификации Н. Н. Матышевой и Л. С. Косицкой. Продукты, реализующие положительные реакции, исключали из диет пациентов. У 100% пациентов выявлены специфические реакции I, III и IY типа на пищевые аллергены, количество реакций возрастало пропорционально тяжести болезни, наибольшее у больных ВЗК, наименьшее - у здоровых лиц. Исключение из рациона питания выявленных аллергенов, а также непищевых аллергенов, поступающих с продуктами питания, способствовало значительному клиническому улучшению, а у больных ЯК наступлению стойкой клинической и эндоскопической ремиссии. Для определения наиболее полного спектра пищевой сенсибилизации у пациентов с БОП необходимо использовать методы оценки клеточного и гуморального типов сенсибилизации совместно. Диета исключения из рациона питания пищевых аллергенов у пациентов с БОП приводит к стойкому клиническому улучшению и длительному поддержанию ремиссии, особенно, у больных ЯК.

Objective: to study the spectrum of food sensitization in patients with diseases of the digestive system (DDS) to adjust their dietary therapy. The laboratory examination and treatment of 210 patient: 185 patients with DDS (gastroesophageal reflux disease, chronic gastroduodenitis, biliary dyskinesia, irritable bowel syndrome, IBD (ulcerative colitis(UC) and Crohn’s disease) and 25 healthy adults. To identify the reactions I, III and IY-type food allergens used a range of methods - ELISA (IgE-specific) and the reaction of inhibition of migration of leukocytes (RIML), modification NN Matyshevoy and LS Kositsky. Products that implement positive reactions, patients were excluded from the diet. 100% of patients identified specific reactions I, III and IY-type food allergy, the number of responses increased in proportion to the severity of the disease, most patients with IBD, the smallest - in healthy individuals. Exclusion from the diet of the identified potential allergens contributed to a significant clinical improvement, and in patients with UC onset of persistent clinical and endoscopic remission. Conclusion: to determine the most complete range of food sensitization in patients with DDS is necessary to use methods for assessing cellular and humoral sensitization types together. Diet exclusion of food allergens from the diet of patients with DDS leads to persistent clinical improvement and long-term maintenance of remission, especially in patients with UC.
